Keep each care setting distinct
CMS describes PACE as a program providing comprehensive medical and social services to certain frail older adults who continue living in the community. A skilled nursing or rehabilitation position may have a different employer, workflow, and clinical team. State the specific setting and service instead of combining them under a broad geriatric-care label.
Clarify whether care is delivered in a clinic, a participant’s home, a facility, or more than one location. Describe the schedule, patient population, travel, on-call or coverage, documentation, and coordination expectations. Those are role requirements candidates can evaluate, not details to leave for the final interview.
Select a discipline for the defined work
A search might involve an adult-gerontology primary-care NP, a PMHNP, a psychiatric RN, a therapist, or another clinician, depending on the actual service. Do not treat age group alone as enough to select a specialty. Identify whether the need is primary care, psychiatric care, nursing, counseling, or care coordination, then ask clinical leadership what education and experience the work calls for.
AANPCB certification information describes the AGNP exam as assessing primary-care knowledge for adolescent through older-adult populations. That can help orient a hiring team, but it does not establish an individual’s full practice authority, organizational qualifications, or readiness for a particular position. Verify the exact role, licensure, certification, and privileges through the employer’s normal process.
Explain the interdisciplinary team
Show how the role coordinates with primary-care clinicians, behavioral-health staff, nursing, rehabilitation, social services, and other partners relevant to the program. Identify who owns clinical decisions and referrals, how information is shared, and what consultation is available. Where patient care involves multiple disciplines, candidates should understand how their contribution fits the plan.
Separate must-have experience from preferred familiarity. An organization can assess transferable experience, but it should be clear about training, orientation, and support. If the team expects a clinician to cover more than one specialty or site, describe how priorities will be set and what work is not part of the position.
Approach the market accurately
Cornerstone considers selected permanent searches in older-adult and post-acute care. We assess the patient population, clinical responsibilities, required experience, and support before agreeing on a search. A role serving older adults should not automatically be treated as a general NP position.
A focused brief with setting, duties, team, credentials, schedule, and location gives CTG a basis to assess the search. If the role fits our targeted scope, the search can then be built around the actual opportunity and the clinicians qualified to consider it.
Verify the specific service before opening the search
Ask the care-program leader to describe the population, service, and daily clinical work in concrete terms. Confirm which discipline and experience fit those responsibilities, who provides consultation, and how the position coordinates with the rest of the team. For a developing search area, this first conversation helps determine whether the scope is specific enough to take to market.
Once the need is clear, share the brief with the clinical and credentialing owners for review. They can confirm the qualifications and practice requirements for that employer and location. Begin outreach with a defined role and a realistic description of the service.
